The following are surgical methods used to obtain sperm when severe oligospermia (low sperm count) or azoospermia (no sperm) are presented by a patient. All of these surgical interventions are used in conjunction with ICSI because very few viable sperm are obtained through these methods.

Percutaneous Epididymal Sperm Aspiration (PESA) or Testicular Sperm Aspiration (TESA)

PESA and TESA are techniques that employs fine needle to aspirate sperm directly from the epididymis or the testis by a fertility specialised urologist. For this procedure, an intravenous conscious sedation is conducted.

A fine needle attached to a syringe is then inserted into the epididymis or the testis. Suction is then applied to the syringe and the fluid that is aspirated is given to the andrologist for examination. If an adequate number of sperm has been found, the surgery is concluded; otherwise, it may take multiple aspirations in different regions of the testis to obtain sperm.

Micro Testicular Sperm Extraction (MESE or Micro-TESE)

Micro-TESE is the most invasive of the surgical procedures for retrieving sperm. It is generally conducted when there is evidence that concludes the affliction of primary spermatogenic failure (total absence of sperm).

Micro-TESE begins with the patient undergoing general anaesthetic. The surgeon (urologist) will then make a small incision in the scrotum and expose the testicle. From this point a microscope is utilized to observe the tissue of the testicle to discover regions where spermatogenesis (sperm production) may be occurring. If a region is found, small tissue samples will be excised from this area and given to the andrologist who then searches through the tissue and isolates viable sperm. These can then be used to fertilize eggs using ICSI.
